Life Insurance and AD&D

The CSU provides Basic Life Insurance and Accidental Death and Dismemberment (AD&D) at no cost to you as an eligible employee. The amount of coverage varies based on employee category.

Basic Life Insurance and AD&D become effective on the first day of the following month from your date of hire or eligibility. For questions, exclusions, limitations, and reductions, beneficiary designations, and claims, please refer to .

Waiver of Premium: Premiums are paid by Cal Poly.
: Upon separation, you may continue coverage at your cost.
: Offers aid before/during trips, including visa information, health recommendations, and connection to medical providers.
: Provides resources for financial planning, estate planning assistance, health and wellness, funeral arrangements.

The CSU recognizes that individuals have diverse needs and has provided eligible employees with the opportunity to �� for the right amount of protection for you and your family. This is in addition to any employer-paid basic life, accidental death and dismemberment insurance already provided.

Employees may �� for Voluntary Life and Accidental Death & Dismemberment within 60 days of their hire date with no medical certification required for up to the guaranteed issue amounts of $150,000 for employees, $50,000 for spouses/domestic partners and $20,00 for dependent children.
